Longitudinal Geriatrics Training

Cherry Hill’s Longitudinal Geriatrics Training (LGT) is our longitudinal geriatrics curriculum that has been developed to provide ‘long term care’ in the sites where our patients will reap the most benefit: in their own homes.

Beginning in the resident’s second year and until graduation, residents act essentially as physician case managers for two of their own geriatric continuity patients and engage with them through regular home visits, office visits, and telephone encounters. Residents closely coordinate care of their patients through transitions between home, hospital, rehab, or hospice.

Didactics, held quarterly, include case presentations and lectures on the outpatient management of geriatric syndromes. These sessions are led by our family medicine faculty with additional geriatric certification. Additionally, this longitudinal curriculum interfaces with the R2 geriatric rotation allowing for more one-on-one coaching and case review.

Palliative Care

Palliative care is an important part of LGT and our residency’s medical practice. For residents who want to learn more, we have an established inpatient and outpatient palliative care elective rotation as well as the popular Palliative Care AOC.
